
Y.S. Rajashekar Reddy the former chief minister of Andhra Pradesh was killed in a helicopter crash on September 2nd 2009. A four member committee was formed to find the route cause for the accident in which four people along with pilot died. The committee said that the accident was caused by distracted pilots going through flight manual and losing control over the Bell-430 machine. Pilots may have erred in evidence thrown up by in-flight instruments, hinted by panel led by R.K.Tyagi. “The crew noticed a snag and was engrossed for vital six minutes before the impact in searching for the relevant checklist from the flight manual. This distracted their attention leading into the loss of situational awareness” said by the report submitted by them. The pilots should have relied more on visuals rather than onboard instruments said by another official. The whether was unfit for flying and the chopper was caught in a vortex of cloud formation which behaves like a whirl wind. The helicopter YSR was flying was a seven seated, double engine which got disappeared over the Nallamala forests in heavy rain on his way to chittor from Hyderabad. The chopper was found 24 hours later at Rudrakonda hill near Kurnool.
